・福井県初の複数医師・24時間体制の在宅医療専門クリニック「オレンジホームケアクリニック」
・Started「Orange Home Care Clinic」which is Fukui Prefecture’s first multi-doctor, 24-hour home health care clinic

2012〜

・医療的ケア児の地域の居場所「OrangeKids'CareLab.」@福井市
・Started a place for children who need medical care 「OrangeKids'CareLab.」

2013〜

・「みんなの保健室」@福井市新栄、宝永
・Started a「Health-cultural centers」where residents can connect and care for each other as a place to live in the community.

2014〜

・訪問看護「地域看護ステーションみかんの木」@福井市田原町
・Started a home visit nursing「Mikan no ki」

・訪問介護「ライフサポートチームこれでいいのだ。」@福井市田原町
・Started a home visit care「Life support team Korede iinoda」

・Launched a special experinces in Karuizawa town for children who need medical care「Karuizawa Kids'CareLab.」

・Completion and relocation of 「Orange Living Base」, home of the 「Orange Home Care Clinic」

・ケアの文化拠点「診療所と大きな台所のあるところほっちのロッヂ」@長野県軽井沢町
・Started a cultural center of care(An outpatient and home care clinic and welcome kitchen)「Hotch no Lodge」

2020〜

・医療的ケア児の地域の居場所「OrangeKids’CareLab.」新築移転＋「まあるカフェ」オープン @福井市灯明寺
・Newly built and relocated 「OrangeKids’CareLab.」, a local place for children with medical care, and opening of 「Maaru Cafe」

2021〜

・補装具・日常生活用具「チェアラボ金沢」 @石川県金沢市
・ Started a team of seating and positioning specialists「Chair Lab Kanazawa」

・地域支援型診療所「勝山オレンジクリニック」@福井県勝山市
・ Started an outpatient and home care clinic「Katsuyama Orange Clinic」

・特別養護老人ホーム「さくら荘」デイサービス「さくらんぼ」（社会福祉法人勝山福祉会）@福井県勝山市
・ Intergrated and renovated a special elderly nursing home「Sakura-so」& Day care「Sakuranbo」

2022〜

複合型拠点つながるベース内「つながるクリニック」に加えて、「ROOT × ROUTE」「みいつカフェ」開業@福井市二宮
・Renovated the「Tunagaru Clinic」, integrated with cafe「Meets cafe」, and a personal gym「ROOT × ROUTE」

・「ほっちのロッヂ」が第10回アジア太平洋地域・高齢者ケアイノベーションアワード2022・Social engagement program部門にて日本初・最優秀賞を受賞
・「Hotch no Lodge」Asia Pacific Eldercare innovation Awards 2022 / Winner of Innovation of the year -Social Engagement Program
